In order to most effectively serve our residents and business owners, the Yaphank Fire District compiles pre-plan information about commercial and industrial buildings within Yaphank, and Medford Area. These forms are NOT a substitute for an inspection for legal reasons, they are simply a way for the District to gather information about a given building prior to the need for our response in an emergency. Having this information on hand allows us to mitigate any problems that may arise more quickly and efficiently. We thank you for your time in completing this form.
